When you ask us to get in touch
Our forms collect the name, work email, business name, optional city, and service interest you provide. We also record which form you used, the submission time, and the wording and version of your contact consent. A request from the HIPAA tool identifies that you reached its results page; it does not include your answers, score, or report.
When you check the consent box and submit, you ask Clinic Care Tech to contact you by email about your request and relevant IT services. You can ask us to stop at any time by replying to an email or writing to christian.gonzalez@cliniccaretech.com. We do not sell the details submitted through these forms.
How requests reach us
We use Formspree to receive and store form submissions and deliver notifications to our business inbox. Formspree and our hosting provider may process technical information, such as IP addresses, for delivery, security, and abuse prevention. See Formspree’s privacy policy for its practices.
Your HIPAA checklist
The checklist runs in your browser. Your answers are saved on this device so you can return to them within seven days of your last change. Expired saved answers are removed the next time you open the tool. Use “Start fresh” or “Start Over” to clear them immediately, or clear your browser’s site data. PDF reports are generated on your device. We do not receive your answers or PDF when you use or download the checklist.
The tool is available without providing contact details. The optional follow-up form is separate from your checklist. Please do not enter patient information or other protected health information in any website form.
Cookies, analytics, and external links
This website does not install advertising trackers or visitor analytics scripts. The checklist uses local browser storage for the saved answers described above. Our hosting provider may maintain server logs. Booking links open Calendly, which handles information under its own privacy policy. We do not automatically send your checklist answers to Calendly.
Questions, access, or deletion
To ask about your information, request a correction or deletion, or stop follow-up emails, contact christian.gonzalez@cliniccaretech.com. We use submitted contact details to respond to requests and manage business relationships. Requests concerning copies held by service providers will be handled through the relevant provider.
